Zenith Vision Development Center (ZVDC) is an out-of-network provider, which means that we do not bill or accept payment from insurance directly.
We believe it is important to provide a comprehensive evaluation for each client. Insurance companies set very specific requirements about which tests can be administered and how much time can be spent on specific portions of the evaluation, which limits the ability to be thorough and comprehensive. In order to provide the best possible service to our patients, using the most up-to-date assessment tools and research, we do not accept insurance or payments directly from insurance companies.
Some insurance companies offer partial reimbursement for functional vision evaluation services and vision therapy. Following your evaluation, we provide an itemized superbill which is the document required by insurance for out-of-network reimbursement. Please be aware that insurance reimbursement varies by provider and plan and not all offer reimbursement. Your insurance company makes coverage decisions and any reimbursement must go directly to you; ZVDC cannot accept it.

Insurance providers may also request the reason for testing or for specific diagnostic codes. Here is a list of common diagnostic codes for functional vision evaluations. This is not an exhaustive list.
• H51.11, Convergence Insufficiency
• H55.82, Deficient Smooth Pursuit Eye Movements
• H55.81, Deficient Saccadic Eye Movements
• H53.2, Diplopia
Common billable services included in a functional vision evaluation with ZVDC: clinical interview, record review, questionnaire analysis and review, functional vision evaluation, analysis of results, formulation of recommendation plan, communication of results and recommendations during a single feedback session, and preparation of written report. Additional services such as follow-up sessions, visual information processing evaluations, and vision therapy are billed separately.
